Location: Hempstead, NY
Job Type: Full-time, starting part-time
Pay: $25–$29 per hour (approximately $27 per hour)

We are seeking a warm, organized, and detail-oriented Spanish-speaking Intake Coordinator to join a care management program serving Long Island. In this role, you’ll help individuals and families navigate services, insurance requirements, referrals, and enrollment.

During your initial months, you’ll also serve as a Care Manager, conducting monthly home visits and providing ongoing support to members and families.

Responsibilities

• Coordinate the intake process, including reviewing referrals, assessing eligibility, and supporting enrollment

• Conduct monthly home visits and provide care management support

• Work with families, healthcare providers, therapists, community organizations, and Medicaid Managed Care Plans

• Conduct assessments and develop individualized care plans

• Maintain accurate, timely documentation

• Follow applicable New York State Department of Health, Health Home, and regulatory requirements

• Attend weekly virtual trainings, team meetings, and program updates

• Help maintain a current directory of services, providers, and community resources to support appropriate referrals

Qualifications

• Bachelor’s degree required

• Spanish bilingual skills required

• Strong organizational, communication, and computer skills

• Detail-oriented, dependable, and able to work independently and as part of a team

• Comfortable communicating with families and working through paperwork

• Strong problem-solving skills and a proactive approach

• Experience in healthcare, social services, psychology, case management, government programs, or insurance is a plus
