About the Role:
Join MedPOINT Management as a CCS Nurse Case Manager, where you will play a vital role in enhancing patient care and outcomes. This position offers an exciting opportunity to work within a dynamic team in Sherman Oaks, CA, dedicated to providing exceptional healthcare management services.


Responsibilities:
  • Conduct comprehensive assessments to develop and implement individualized care plans for patients.
  • Coordinate with healthcare providers, patients, and families to ensure effective communication and care continuity.
  • Monitor patient progress and adjust care plans as needed to optimize health outcomes.
  • Facilitate access to necessary resources and services, including medical, social, and community support.
  • Document all interactions and maintain accurate records in compliance with regulatory standards.
  • Provide education and support to patients and families regarding treatment options and healthcare navigation.
  • Participate in interdisciplinary team meetings to discuss patient cases and share insights.
  • Stay updated on industry trends and best practices in case management and patient care.
Requirements:
  • Active RN license in the state of California; CCS certification preferred.
  • Minimum of 3 years of nursing experience, with a focus on case management or related fields.
  • Strong critical thinking and problem-solving skills to assess complex patient needs.
  • Excellent communication and interpersonal skills for effective collaboration.
  • Proficiency in electronic health records (EHR) and case management software.
  • Ability to work independently and manage multiple cases efficiently.
  • Compassionate demeanor with a patient-centered approach to care.
  • Knowledge of community resources and healthcare systems.
