🩺 Nurse Practitioner – Wound Care & Primary Care (Long-Term Care)

📍Cincinnati, Ohio | 🏥 Skilled Nursing Facility Setting | 🕒 Part-Time (20–24 hrs/week)

We are seeking a dedicated and experienced Nurse Practitioner to provide wound care and primary care services to patients in a long-term care setting. This is a great opportunity for a provider who enjoys working with medically complex patients and collaborating within an interdisciplinary team.

🔹 Responsibilities

• Provide comprehensive wound care and primary care services to patients in skilled nursing facilities
• Diagnose and treat acute and chronic conditions, with a focus on complex wound management
• Prescribe medications, including controlled substances in accordance with Ohio state law
• Order, review, and follow up on labs, diagnostics, and treatment plans
• Collaborate closely with nursing home staff and primary care physicians to ensure continuity of care
• Communicate changes in patient condition and update care plans as needed

🔹 Required Qualifications

• Active, unrestricted Nurse Practitioner license in Ohio
• Board Certification (FNP, AGNP, or related specialty)
• Clinical background in adult, family, or geriatric care
• Wound Care Experience

🔹 Preferred Skills

• Comprehensive patient assessment and diagnosis
• Chronic disease management
• Acute care evaluation and treatment
• Medication management and prescribing

🔹 Schedule & Compensation

• 3 days per week (Between Monday–Friday, 8am-5pm)
• 20–24 hours per week (may start off lower, around 12 hours per week and then increase after 30 days)
• Flexible scheduling (set your own consistent weekly schedule)
