The Home Infusion Pharmacist is responsible for the safe and accurate preparation, dispensing, and monitoring of intravenous (IV) medications administered in a patient’s home or alternate care setting. This role ensures compliance with all regulatory standards while collaborating with healthcare providers to optimize patient outcomes in a home infusion environment.
Key Responsibilities:
- Review and verify physician orders for home infusion therapies, including antibiotics, total parenteral nutrition (TPN), chemotherapy, and specialty biologics
- Compound and supervise the preparation of sterile IV medications in accordance with USP <797> and <800> standards
- Ensure accurate dispensing and labeling of medications and supplies for home delivery
- Provide clinical oversight, including dosing adjustments, therapeutic monitoring, and patient-specific care planning
- Collaborate with physicians, nurses, and case managers to coordinate patient care
- Counsel patients and caregivers on medication administration, storage, side effects, and safety
- Monitor patient outcomes and laboratory results; intervene as necessary
- Maintain compliance with state and federal regulations, accreditation standards, and company policies
- Oversee pharmacy technicians and support staff in compounding and workflow processes
- Participate in on-call rotation for clinical support and urgent patient needs
Qualifications:
- Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy
- Active pharmacist license in the applicable state
- Experience in sterile compounding and home infusion or hospital pharmacy preferred
- Knowledge of USP <797>, USP <800>, and aseptic technique standards
- Strong clinical knowledge of IV therapies and disease states
- Excellent communication and organizational skills
Preferred Qualifications:
- Board certification (e.g., BCPS)
- Experience with home infusion pharmacy services
- Familiarity with electronic medical records and pharmacy management systems
Work Environment:
- Pharmacy cleanroom and office setting
- May include occasional patient interaction (in-person or virtual)
- On-call or weekend rotations may be required
Physical Requirements:
- Ability to stand for extended periods

- Ability to lift and transport medication supplies (up to ~25 lbs)
