 Position Summary:

Responsible for providing safe, effective, appropriate, timely, and cost-efficient medication therapy that complies with hospital policy and procedure and Federal, State and local laws, and any applicable rules and regulations. Responsible for contributing to, developing and maintaining policies and procedures as they relate to medication usage and clinical services and contribution to pharmacy department and hospital initiatives based on strategic goals established by executive team and/or pharmacy supervisors and managers. Additionally, clinical pharmacy specialists serve as preceptor for pharmacy residents and students as scheduled by Residency Program Directors and Residency Advisory Committee. The clinical pharmacy specialist & antimicrobial stewardship leader serves as the designated, qualified and appointed antimicrobial stewardship leader for the hospital.

  1. Qualifications:
    1. Current Active Pharmacy License with Texas State Board of Pharmacy required
    2. Current Texas State Board of Pharmacy preceptor license required
  2. Education:
    1. Bachelor of Science Degree in Pharmacy or Doctor of Pharmacy Degree required
    2. Registered Pharmacist from Texas State Board of Pharmacy required
    3. Specialty certification (e.g., BCPS, BCCCPS) preferred
  3. Training and Experience

 

    1. Completion of a pharmacy residency program or >/=3 years of experience working as a clinical pharmacy specialist in a hospital
    2. Board certification from board of pharmacy specialties or other similar specialty certificate within three years if not already achieved, (e.g., BCPS, BCCCPS)
    3. Board certification in infectious disease, or certification in antibiotic stewardship-related training course within one (1) year of appointment
    4. If primary job function is working in the critical care, internal medicine, trauma and/or emergency departments (>50% of time spent), Advanced Cardiac Life Support (ACLS) training is required as a member of rapid response and code blue teams, (must have or obtain within three (3) months of hiring) and maintain qualifications throughout employment
    5. If primary job function is working in the Emergency Department (>50% of time spent), Advanced Trauma Life Support (ATLS) and Pediatric Advanced Life support (PALS) training is required, (must have or obtain within six (6) months of hiring) and maintain qualifications throughout employment
    6. If primary job function is working in the NICU department (>50% of time spent), Pediatric Advanced Life support (PALS) training is required, (must have or obtain within six (6) months of hiring) and maintain qualifications throughout employment
    7. IV Certification as per USP 797 (must have or obtain within two ( )2 months of hiring) and maintain qualifications throughout employment.
