McGregor PACE is an innovative, community-based Program of All-inclusive Care for the Elderly. Our five senior day centers and clinics are located in the counties of Cuyahoga, Lorain, and Summit.
McGregor PACE is expanding! We have opportunities available for an experienced and compassionate Registered Dietitian to work in a community-based setting with the frail and elderly populations. We have openings in our East Cleveland (44112), Brooklyn (44144), and Elyria (44035) locations.
McGregor PACE takes great pride in the provision of comprehensive services for frail older persons. The Dietitian provides direct participant care to program enrollees; develops and implements nutritional care plans, communicates plans, provides education to participants, family, and staff as needed, establishes a follow-up system and maintains documentation. The Dietitian works with an Interdisciplinary Team to develop plans of care that meet the participants' needs.
The workplace setting includes our PACE senior day centers and clinics, and home visits which may include visiting assisted living facilities, long-term care facilities, group homes, and community homes. There is also flexibility for remote work!
Qualifications include:
- Registered Dietitian; licensed as a clinical dietitian in the state of Ohio
- Bachelor’s degree in nutrition and dietetics, required; master’s degree, preferred
- Minimum of one year’s experience working with a frail or elderly population, required
- Experience in community-based programs and interdisciplinary care settings, desirable
- Serv Safe Certification desired
- Current Ohio driver's license and reliable transportation, required
