Join Mazzoni Center, Philadelphia’s leader in LGBTQ health and wellness, as our next Case Management Services Manager. We're seeking a passionate, community-focused leader to help drive transformative outreach programs that directly support LGBTQ individuals, youth, people living with HIV, and other underserved communities.
What You’ll Do:
As our Case Management Services Manager, you'll be the heartbeat of our case services department—coordinating with external partners, leading program reporting efforts, and supervising a talented team of Medical Case Managers. You’ll:
- Oversee program reporting (AACO Housing, DEFA, PHMC Token, and more)
- Supervise and evaluate Case Managers and Support Staff
- Monitor referrals and manage intake workflows
- Represent the department in quality assurance and advisory meetings
- Identify staff training needs and support professional development
- Handle HIPAA-compliant documentation and administrative tasks
- Collaborate closely with external stakeholders and internal teams
Leadership & Supervision:
- Directly supervise Medical Case Managers
- Conduct supervision, performance evaluations, and case reviews
- Onboard and train new staff
- Support interdisciplinary coordination across services
Requirements:
- Master’s degree in social work, psychology, counseling, therapy, or related field
- Minimum of 2 years of case management experience in a medical or social service setting
- Supervisory experience required
- Experience with HIV/AIDS services preferred
- Strong interpersonal, communication, and organizational skills
- Knowledge of HIPAA, Pennsylvania Act 148, and confidentiality policies
Bonus Points If You Have:
- Bilingual skills (especially in Spanish or other relevant languages)
- Experience with database systems (CareWare/eShareNet)
- Familiarity with Philadelphia-area service networks
