Role Description
This is a full-time, on-site/on-the-road role located in Philadelphia, PA, 19131 for a Case Manager. The Case Manager will oversee client caseloads, provide individualized support services, develop tailored care plans, and monitor progress to ensure client needs are met. The role includes coordinating with families, community agencies, and service providers, while maintaining accurate documentation and ensuring compliance with organizational policies and relevant regulations.
