Position Summary:
The Medical Case Management Supervisor is responsible for supporting the daily operations of the Medical Case Management program. This role includes supervising case managers, supporting quality improvement initiatives, and working closely with internal staff and external partners to enhance program outcomes and client services. The ideal candidate is a compassionate and experienced professional with a strong background in social services, team leadership, and care coordination for individuals living with HIV/AIDS.
Key Responsibilities:
Provide direct supervision to Medical Case Managers, including weekly individual supervision and documentation.
Conduct performance evaluations and chart reviews (Care Ware, athenaNet, and hard files) for supervisees.
Assist with the recruitment, orientation, and training of new staff.
Support staff with client-related crises and complex case management needs.
Complete monthly program reporting and assist with Quality Management initiatives.
Collaborate with external agencies and community providers to enhance client care and program development.
Help oversee daily program operations and contribute to ongoing program planning and evaluation.
Qualifications:
Master’s degree in a social service field (e.g., social work, counseling, psychology, therapy).
At least 2 years of experience in social work, medical case management, or a related field.
Supervisory skills and experience managing staff or leading teams.
Knowledge of HIV/AIDS care, LGBTQ issues, and available community resources.
Strong oral and written communication skills.
Proven ability to manage multiple responsibilities and deadlines.
Commitment to maintaining confidentiality and ethical standards in client care.
Bilingual (Spanish/English) skills are a plus.
Physical Requirements:
Ability to travel throughout the city regularly.
Ability to communicate effectively in written, verbal, or signed language.
Ability to lift up to 35 lbs.
Ability to work at a computer for extended periods (up to 6 hours/day).
Ability to work in a moderate-noise, open office environment.
Compliance with all vaccination requirements as per Mazzoni Center policies.

- Background checks are required, including Criminal Background Clearance, Child Abuse Clearance, and FBI Clearance.
