What are the responsibilities and job description for the Fatherhood Program Facilitator / Case Manager position at MAUMEE VALLEY GUIDANCE CENTER?
Maumee Valley Guidance Center is seeking a motivated and compassionate Fatherhood Program Facilitator / Case Manager to support our Fatherhood Initiative Grant program.
This position will work directly with fathers to provide parenting education, facilitate the dads program, and offer case management services to help participants strengthen relationships with their children, improve family stability, and address barriers such as employment, housing, transportation, childcare, benefits, and other needs.
The ideal candidate will be comfortable leading groups, building positive relationships with fathers, coordinating community resources, and completing grant-required documentation.
Primary Responsibilities
- Facilitate fatherhood programming for participating dads.
- Provide individualized case management and goal planning.
- Complete intakes, assessments, action plans, progress notes, and required grant documentation.
- Help fathers connect with community resources and support services.
- Assist with outreach, recruitment, and participant follow-up.
- Collaborate with community partners and referral sources.
- Support fathers in improving parenting skills, family relationships, and overall stability
- Complete required assessments, action plans, progress notes, attendance records, referrals, and outcome data.
- Assist with grant reporting, data collection, and program evaluation..
Preferred Qualifications
- Bachelor’s degree in social work, human services, counseling, psychology, education, criminal justice, or a related field preferred.
- Experience in case management, family services, parenting education, group facilitation, outreach, or grant-funded programming preferred.
- Strong communication, organizational, and documentation skills.
- Ability to work respectfully with fathers and families from diverse backgrounds.
- Valid driver’s license and reliable transportation required.
This is a grant-funded position. Some flexible hours may be required to meet participant needs.
