RN / Field Case Manager – Leadership Track | Confidential Home Health Agency

📍 San Jose & Bay Area (Los Gatos, Palo Alto, Atherton, Campbell, Saratoga, CA)

💼 Full-Time-Monday to Friday, 9am-5pm with some evening or weekend in urgent cases only. | $140,000 – $160,000 Performance-Based Bonuses, Sky is the limit!!!

Position Summary

We are seeking an experienced Registered Nurse (RN) Field Case Manager to join a 5-Star Medicare-rated, clinician-led home health agency. This role is ideal for an experienced and motivated RN who enjoys direct patient care and wants to grow into a leadership track role. You will coordinate patient care, mentor field clinicians, and ensure clinical excellence across every episode of care.

Key Responsibilities

• Perform comprehensive patient assessments and develop individualized care plans.
• Serve as the primary clinical liaison between care teams, patients, and leadership.
• Provide in-home skilled nursing care and ensure timely, accurate documentation.
• Mentor and support field nurses and therapists to promote clinical consistency.
• Identify early signs of decline to prevent hospitalizations and improve outcomes.
• Coordinate care with interdisciplinary teams (PT, OT, ST, MSW, HHA) and referring providers.

Must-Have Qualifications

✅ Active California RN license (required)

✅ Minimum 3 years of RN experience (non-negotiable)

✅ Home Health field experience (required)

✅ Leadership experience or demonstrated leadership potential (required)

✅ Strong clinical judgment, organizational, and communication skills

✅ Valid driver’s license, auto insurance, and reliable transportation

💬 Fluency in Mandarin or Vietnamese a plus

Compensation & Benefits

💰 Base Salary: $140,000 – $160,000/year (DOE)

🎯 Performance-Based Bonuses available for clinical quality and care coordination ($2,000 /month)

🚗 Mileage reimbursement or company vehicle option
