Two positions available:
Full-time; 7:30am – 5:30pm (9.5 hour days; 8 shifts per pay period); including every other weekend
Part-time; 7:30am – 5:30pm (9.5 hour days; 6 shifts per pay period); including every other weekend
JOB SUMMARY:
Responsible and accountable for observing, monitoring, and performing tasks needed to promote the patient’s optimal level of well-being and function. Under the direction of the professional nurse, performs assigned direct health care in a patient’s home, provides assistance to families, and communicates information to the care coordinator and team members.
ESSENTIAL JOB FUNCTIONS:
· Collects objective data to assist the RN Case Manager in formulating an individualized plan of care.
· Implements plan of care in accordance with policies and procedures; performs nursing tasks as assigned.
· Participate in performance improvement activities.
· Observe, report and document data to evaluate patient responses to care; progress toward outcomes; and potential changes to the patient care plan.
· Implement the advocacy role through activities that inform, support, and affirm the client and family’s self-determination.
· Promote continuity of care through communication with the health care team.
· Provide direct and indirect care for the patient, as appropriate.
· Monitor and support patient and caregiver participation in furthering the goals of patient care.
· Report ethical issues and participate with patients, families, and the multidisciplinary staff to explore options and achieve resolutions.
· Abide by the Compliance Program at Masonic Care Community ensuring that all information is kept confidential, and is respectful of the Patient Bill of Rights.
· Perform other tasks/duties as assigned.
JOB QUALIFICATIONS:
· Graduation from an accredited practical nursing program and current license by New York State Board of Nursing.
· At least one year acute care experience.
· Valid New York State driver’s license, and/or access to dependable transportation.
The base range for this position is $28.25 - $31.50 per hour commensurate with work location, experience, skills, certifications, education & prior accomplishments.
The position may be eligible for additional compensation not listed here, including but not limited to shift differentials, incentives or bonuses, as applicable.
