LPN

Position Scope:  Under the direction of the Unit Manager/Supervisor, will assist with day to day clinical management, medication administration, and treatment administration

Responsibilities:

• Provide, assist, and monitor other team members to provide high quality nursing care using a holistic and person centered approach.
• Be sensitive to the changing and ongoing needs of residents, families, and other caregivers.
• Monitor care delivered to ensure the plan of care is being upheld and remains appropriate to resident needs.
• Follows facility policy and procedure for clinical and infection control practices.
• Liaise with medical and other members of the interdisciplinary team to ensure quality care.
• Provide support to the Unit Manager/Supervisor, bringing to their attention any matters they should be aware of e.g., accident and incidents, change in resident condition, etc.
• Contribute to the implementation and ongoing adherence to clinical protocols and standards of care.
• Provide therapeutic treatments and procedures as ordered by medical staff e.g., medication administration, wound care, obtaining lab specimens, etc.
• Update the appropriate medical staff with change in resident condition, laboratory results, resident/family concerns, etc.
• Update family/responsible party with change in resident conditions, incident and accidents, etc.
• Follow up with acutely ill residents, residents with a recent incident, and those with change in condition on a daily basis to ensure improvement. If decline is noted update medical and Supervisor.
• Communicates and collaborates with other members of the interdisciplinary team to maintain quality care.
• Participate in Team Member meetings.
• Maintain medical records in accordance with facility policy.
• Assist in maintaining unit equipment, supplies, and inventory.
• Promptly report equipment/maintenance problems to relevant individuals.
• Attends in-services to keep current, knowledgeable, and remain compliant with requirement of mandatories.
• On any scheduled shift, you may be required to work past your regularly scheduled hours as directed by your Supervisor.
• Other duties as assigned and within scope of practice.

Qualifications/Education/Experience:
 

• Graduate of an accredited nursing curriculum 
• Current NYS Registered Professional Nurse License (RN)/Licensed Practical Nurse (LPN)

Salary : $29 - $32
