- Every 1st and 3rd Saturday of the month

- Evaluate, diagnose, and treat acute and chronic medical conditions
- Perform preventive care visits, follow-ups, and routine health maintenance
- Develop treatment plans and coordinate care with clinical support staff and specialists as needed
- Maintain accurate and timely documentation in the EMR
- Educate patients on treatment plans, preventive health, and disease management
- Collaborate with physicians, advanced practice providers, behavioral health professionals, and support staff to ensure coordinated patient care
Salary : $140 - $145
