Position Summary
The Adolescent Case Manager provides supportive case management services to adolescents receiving treatment for substance use and co-occurring disorders. This position works collaboratively with the treatment team, families, schools, and community partners to ensure youth have access to appropriate clinical, medical, educational, and social support services. The Case Manager assists in reducing barriers to care, enhancing recovery outcomes, and supporting long-term wellness.
Essential Duties & Responsibilities
- Conduct intake interviews and psychosocial assessments to identify client needs.
- Assist with the development and implementation of individualized service or treatment plans.
- Coordinate referrals to medical, psychiatric, mental health, educational, vocational, housing, and community resources.
- Provide case management support, including scheduling, follow-up, and monitoring of services.
- Collaborate with counselors, therapists, and multidisciplinary team members to track progress and update care plans.
- Facilitate communication between youth, families, schools, probation officers, and other service providers.
- Provide education and support to families regarding adolescent substance use and recovery resources.
- Document all contacts, services, and referrals in the electronic health record (EHR) in compliance with agency and state requirements.
- Participate in case conferences, staff meetings, and training related to adolescent substance use treatment.
- Assist clients in navigating barriers such as transportation, insurance, and access to services.
- Support discharge planning and aftercare linkage to community-based supports and recovery programs.
Qualifications & Requirements
Education:
- Bachelor’s degree in Social Work, Psychology, Counseling, Human Services, or related behavioral health field (required).
Experience:
- Minimum of 1 year of experience working with adolescents in substance use, behavioral health, or social services preferred.
- Experience with at-risk youth, case management, or community-based programs a plus.
Knowledge & Skills:
- Understanding of adolescent development, substance use disorders, and co-occurring conditions.
- Knowledge of community resources, support services, and recovery-oriented systems of care.
- Strong organizational, time management, and documentation skills.
- Ability to engage youth and families in a supportive, nonjudgmental manner.
- Bilingual (English/Spanish) preferred but not required.
Other Requirements:
- Valid NJ Driver’s License and reliable transportation (for client transport and community-based work).
Job Type: Full-time
Pay: $52,000.00 per year
