Function
- Actively promote the agency philosophy, mission, strategy, and goals.
- Provide support to Program Director in executing program requirements.
- Act as a liaison for the agency; build excellent collaborative relationships with the provider community and other stakeholders.
Primary Program Responsibility:
- Develop collaborative working relationships with the provider community.
- Assist with the coordinating and hosting regular provider meetings.
- Act as a liaison between Program Director and providers to support mediation and resolution of issues, and the development of strategies to improve shared service delivery.
- Support with identifying gaps in service delivery; provide Program Director with recommendations for resolution that meet requirements while maintaining a person-centered approach.
- Foster partnerships that will help meet agency goals and expand the resource base for persons served.
- Maintain tracking of workflow for all assigned projects/tasks and update designated databases timely to maximize the accuracy of information.
- Identity program trends and make recommendations for process/quality improvement.
- Work with the agency Management Team to promote a positive, productive work environment.
- Act as a liaison between Coordinator's of Community Services (CCS), Team Leader (TL) and providers as needed to mitigate delays/communication gaps and obtain what is needed to execute Person Centered Plans and other core task completion.
- Support Program Director with TL/CCS follow up on status of identified action items.
- Occasionally assist with coordinating and executing core task completion including task entry and billing.
- Perform other duties as requested from time to time that are not specifically listed but are consistent with the position and necessary for efficient and effective operation of the agency.
