Title: Registered Nurse (RN) – Home Health
Location: 2000 Springer Dr, Lombard, IL 60148
Tentative Start Date: Dec 29, 2025
Contract Duration: 13 Weeks
Shift: 8-hour Day Shift (40 hours)
- Schedule: Monday–Friday, 08:00–17:00
- Guaranteed Hours: 40 hours/week
- Onsite Role: Yes
Weekend Requirement: Every 3rd weekend
Holiday Requirement: As needed
On-Call: None
Floating Requirements: Within coverage area; possible neighboring team (not typical)
Position Overview
This Home Health RN role supports patients with multiple comorbidities, commonly including:
- CHF, COPD, pneumonia
- Cancer
- Dementia
- Hypertension
- Kidney disease
- Fractures, falls
- Post-surgical recovery
- Infections, cellulitis
- Wound care, ostomy care
- IV infusions
Documentation is completed in EPIC EMR, with additional use of Sapphire for IV-related tasks.
Patients are located across the Continuing Health PSA, including:
- Coverage Area: Norridge, Harwood Heights, and the Chicago ZIP codes 60656, 60634, and 60641.
RN must have reliable transportation for in-person patient visits.
Key Responsibilities
• Conduct full patient assessments and develop care plans
• Perform wound care: wound vacs, ostomy care, pleural drains
• Administer IV therapy: elastomeric pumps, IV pumps, central line care
• Perform peripheral blood draws
• Complete EPIC charting and documentation
• Provide patient/family education on disease management
• Collaborate with interdisciplinary team members
• Manage caseload independently while prioritizing safety
Work Environment & Expectations
• Scrubs: Any solid color
• Traveler must be comfortable driving daily
• No facility call-backs
• Clinical Interview Required: No
Required Skills & Experience
• Minimum 1 year Home Health RN experience (Must Have)
• Active IL RN License
• BLS (AHA) required
• Strong wound care experience (wound vacs, pleural drains, ostomy care)
• IV therapy central line care skills
• Excellent assessment and documentation skills
• Ability to work independently in the field
• EPIC EMR experience preferred but not required
Salary : $70 - $73
