Position Overview
The Home Health Office Nurse serves as a central clinical point of contact in the office, supporting field clinicians, patients, families, physicians, and the leadership team. Responsibilities
- Coordinate patient care across disciplines (skilled nursing, PT/OT/ST, MSW, HHA) to ensure continuity, timeliness, and adherence to the Plan of Care.
- Triage incoming patient calls, clinician calls, and physician communications; assess urgency, escalate appropriately, and document outcomes.
- Manage lab orders, results review, and follow-up—communicate critical and abnormal results to the case manager and ordering physician, and ensure documentation is closed out in the EMR.
- Support clinical leadership with day-to-day clinical operations, including assisting with care plan oversight, visit utilization tracking, and follow-up on outstanding orders, signed POCs, and physician communications.
- Step in to support intake and staffing operations as needed — including processing referrals, verifying insurance and eligibility, scheduling SOC visits, and assisting the scheduling team with clinician assignments.
- Serve as a clinical resource for field staff on documentation questions, supply needs, infection control, medication reconciliation, and patient-specific concerns.
- Communicate professionally and clearly with physicians' offices, hospitals, SNFs, and other referral sources.
- Support quality and compliance initiatives, including responding to documentation gaps and assisting with chart follow-up under the direction of clinical leadership.
- Maintain accurate, timely, and HIPAA-compliant documentation in the EMR.
- Perform other clinical and operational duties as assigned to support the team.
Required
- Active Illinois RN or LPN/LVN license in good standing.
- Minimum 2 years of home health experience.
- Proven hands-on experience with Kinnser / WellSky EMR.
- Strong working knowledge of Medicare home health requirements and the home health workflow (referral → SOC → recert → discharge).
- Ability to work effectively under pressure, manage competing priorities, and meet deadlines without compromising accuracy or patient safety.
- Demonstrated commitment to quality of care and patient safety.
- Self-starter who takes initiative and ownership; reliable follow-through on assigned tasks.
- Fast learner — comfortable picking up new processes, tools, and payer requirements quickly.
- Excellent communication, organizational, and time-management skills.
- Positive, professional demeanor and the ability to collaborate effectively with clinicians, office staff, physicians, and families.
- Familiarity with OASIS-E concepts and home health documentation standards.
- Working knowledge of ICD-10 coding.
- Familiarity with Medicare, Medicare Advantage, Medicaid, and PPO/HMO plan requirements.
- Awareness of IDPH, CHAP, and CMS survey expectations.
- Prior exposure to intake, scheduling, or care-coordination workflows.
