Candidates must reside in Miami Dade County, FL (Doral, Coral Gables).
We are seeking a dedicated and compassionate Case Manager to join our dynamic team.
Approximately 75% Field Travel within the region seeing Members at home, in assisted living facilities and nursing homes and rest of the time- remote work from home role
Duties
- Responsible for telephonically and/or face to face assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member’s overall wellness
- Effectively manage a caseload that includes supportive and medically complex members
- Determine appropriate services and supports due to member’s health needs
Requirements
- Bachelor's degree in Social Work, Psychology, Counseling, or a related field;
- Bilingual Spanish/English REQUIRED (both reading and speaking and writing)
- Case management experience required
Job Type: Contract
Pay: $29.00 - $36.91 per hour
Expected hours: 40 per week
Work Location: On the road
