The Social Worker plays a vital role in supporting residents and their families by addressing psychosocial needs, facilitating care planning, and ensuring compliance with state and federal regulations. This position is key to enhancing resident well-being, promoting quality of life, and supporting smooth transitions of care.
Key Responsibilities
- Complete psychosocial assessments for residents upon admission and as needed
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Provide counseling and emotional support to residents and families
- Assist with discharge planning, referrals, and coordination of community resources
- Advocate for resident rights and ensure compliance with regulatory standards
- Facilitate family meetings and care conferences
- Support residents with adjustment to illness, disability, and long-term care placement
- Maintain accurate and timely documentation in accordance with facility policy and regulations
- Assist with grievance resolution and conflict mediation
- Participate in QAPI and facility improvement initiatives
- Bachelor’s degree in Related Field
- Current state licensure or eligibility (if applicable)
- Experience in long-term care, skilled nursing, or healthcare setting preferred
- Strong communication, organizational, and interpersonal skills
- Knowledge of federal and state long-term care regulations preferred
- Ability to work collaboratively within an interdisciplinary team
