Director of Social Work

Full Time

Our client, a leading senior living facility located in Southern Westchester County, is seeking an experienced and compassionate Director of Social Work to lead the Social Services department. This role is responsible for overseeing all aspects of psychosocial care, ensuring regulatory compliance, supporting residents and families, and supervising Social Work staff.

The facility is a premier Skilled Nursing Facility, known for its commitment to exceptional person-centered care, innovation, and a supportive community environment.

Key Responsibilities Include:

  • Lead and manage the Social Services department, including supervision and development of social work staff
  • Maintain an assigned caseload of residents, including psychosocial assessments, care planning, and ongoing support
  • Ensure compliance with all federal, state, and local regulations, including CMS and NYSDOH requirements
  • Provide guidance and support to residents and families regarding adjustment, discharge planning, and community resources
  • Participate in interdisciplinary team meetings and care plan conferences
  • Address resident concerns and grievances, ensuring resident rights are upheld
  • Collaborate with leadership to drive quality improvement initiatives and enhance resident experience
  • Maintain timely, accurate, and compliant documentation

Position Requirements:

  • Master's Degree in Social Work (MSW) required
  • Current New York State licensure (LMSW or LCSW) preferred
  • 5 years of experience as a Social Worker in a healthcare setting, preferably in skilled nursing or long-term care
  • Prior leadership or supervisory experience strongly preferred
  • Strong knowledge of CMS regulations, NYSDOH standards, and discharge planning processes
  • Ability to balance leadership responsibilities with a direct service caseload
  • Excellent interpersonal, communication, and organizational skills
  • Must be fully vaccinated against COVID-19

Compensation & Benefits

Salary : $85,000 - $90,000
