Caton Park is actively seeking Home Health Aides to serve as Patient Escorts for our Skilled Nursing Facility in Brooklyn, NY.

Caton Park Rehabilitation & Nursing Center is located directly across from the Parade Grounds at Prospect Park in Brooklyn, NY. The location of our facility takes advantage of being in the heart of Brooklyn and is close to public transportation.

DUTIES:

  • Escort residents to and from appointments
  • Observing and monitoring Residents.
  • Reporting any health issues to the supervising nurse.
  • Maintain confidentiality of all Resident & Facility data.


REQUIREMENTS:

  • Valid New York State HHA Certification.
  • Occasional heavy lifting may be required.
  • Ability to follow the Plan of Care precisely!
  • Must be able to work as a team member
  • Valid photo id
