JOB SUMMARY**: The Per Diem Nurse Practitioner or Physician provides inpatient secure withdrawal management, medical stabilization, and treatment services to adults with substance use disorders. This role includes scheduled clinical coverage as well as overnight and weekend on-call medical support, ensuring continuity of care, timely clinical decision-making, and appropriate escalation for patients admitted to ShoqweL Ya' LhaoLh-ew'xw, Lummi Nation's Secure Withdrawal Management and Stabilization (SWMS) program.
Providers practice within Washington State scope of practice and program protocols under the clinical oversight of the Medical Director.
ESSENTIAL JOB DUTIES AND RESPONSIBILITIES** include the following, and other related duties as assigned.

[ ]{times="" new="" roman",="" times,="" serif"=""}Conduct comprehensive medical assessments including history and physical examinations for voluntary and involuntary admissions.
[ ]{times="" new="" roman",="" times,="" serif"=""}Diagnose and manage acute and chronic medical conditions encountered during withdrawal management and stabilization.
[ ]{times="" new="" roman",="" times,="" serif"=""}Prescribe, manage, and adjust medications used in withdrawal management including medications for opioid, alcohol, sedative-hypnotic, and stimulant withdrawal in accordance with program protocols and Medical Director oversight.
[ ]{times="" new="" roman",="" times,="" serif"=""}Monitor and manage medications for opioid use disorder (MOUD), including assessment of response, adverse effects, and stabilization planning.
[ ]{times="" new="" roman",="" times,="" serif"=""}Provide ongoing medical evaluation and treatment for patients admitted to the SWMS program.
[ ]{times="" new="" roman",="" times,="" serif"=""}Maintain clear, concise, and timely medical documentation in the electronic health record.
[ ]{times="" new="" roman",="" times,="" serif"=""}Collaborate with the Medical Director, nursing staff, behavioral health providers, and support personnel to ensure coordinated inpatient care.
[ ]{times="" new="" roman",="" times,="" serif"=""}Serve as a clinical resource to nursing staff regarding withdrawal progression, symptom management, medication protocols, and clinical decision-making.
[ ]{times="" new="" roman",="" times,="" serif"=""}Identify medical or psychiatric instability requiring escalation and initiate appropriate response including Rapid Response activation, Code Blue procedures, or EMS transfer when clinically indicated.
[ ]{times="" new="" roman",="" times,="" serif"=""}Participate in interdisciplinary staff meetings, case consultations, and care coordination as requested.
[ ]{times="" new="" roman",="" times,="" serif"=""}Provide clinical consultation and guidance to nursing staff regarding changes in patient status, medication management, and withdrawal complications.
[ ]{times="" new="" roman",="" times,="" serif"=""}Participate in the SWMS rotating on-call schedule to provide after-hours clinical support including telephone consultation, remote chart review, order entry, and guidance to nursing staff.
[ ]{times="" new="" roman",="" times,="" serif"=""}Respond to after-hours clinical concerns in a timely manner and determine when in-person evaluation
