Physical Therapist (PT) – Full-Time

LTC Therapy is seeking a Physical Therapist (PT) to join our rehabilitation team in Worcester, MA. This full-time opportunity is ideal for a clinician who enjoys helping residents regain mobility, improve physical function, and work toward greater independence in a skilled nursing and post-acute care environment.
What You'll Do
- Perform physical therapy evaluations and develop individualized plans of care
- Provide skilled treatment focused on strength, balance, gait, transfers, endurance, and functional mobility
- Establish measurable rehabilitation goals and monitor resident progress
- Adjust treatment approaches based on clinical findings and response to therapy
- Educate residents and caregivers on exercises, mobility techniques, and safety strategies
- Complete accurate and timely clinical documentation
- Collaborate with PTAs, OT, SLP, nursing, and other members of the interdisciplinary team

Qualifications
- Active Massachusetts Physical Therapist license required
- Graduate of an accredited Physical Therapy program
- Strong clinical judgment, communication, and teamwork skills
- Skilled nursing or rehabilitation experience preferred, but not required
- New graduates are welcome to apply
