Overview
Are you passionate about connecting individuals with quality in-home care while building relationships throughout the community? Loving Hearts of LA is seeking a motivated Marketing & Community Relations Specialist to assist in generating referrals, strengthening community partnerships, and coordinating participant intake from initial inquiry through service enrollment.
Position Summary: The Marketing & Community Relations Specialist will serve as the brand ambassador promoting the agency’s Home and Community-Based Services (HCBS), Private Pay, and Veterans Affairs (VA) programs. This position will serve as the first point of contact for referral sources, veterans, families, and prospective clients, building strong community partnerships that increase awareness of the agency and cultivates its mission of helping individuals live safely and independently in their homes.
Responsibilities
· Build and maintain referral relationships with hospitals, physicians, rehabilitation centers, discharge planners, support coordinators, social workers, VA representatives, senior centers, assisted living communities, and community agencies.
· Identify and cultivate new referral opportunities through networking, community outreach, and relationship development.
· Represent the agency at health fairs, community events, professional meetings, and educational presentations.
· Coordinate participant intake, explain available services, verify eligibility, and facilitate timely enrollments, coordinating admissions with internal staff.
· Educate referral sources and prospective participants about Medicaid Waiver, Private Pay, and VA-funded home care services.
· Maintain accurate referral, intake, and marketing records while tracking referral outcomes and growth opportunities.
· Prepare marketing materials, assist with social media, and maintain a positive public image for the agency.
· Collaborate with agency leadership to achieve census growth and improved community partnerships.
· Ensure all intake activities comply with Medicaid, VA, Loving Hearts and regulatory requirements.
Qualifications
· Bachelor’s degree preferred; equivalent experience considered.
· Experience working with Medicaid, HCBS, home health care, or VA programs is highly desirable
· Excellent communication, networking, and organizational skills
· Ability to manage multiple priorities while delivering exceptional customer service
· Strong organizational, presentation, and time-management abilities
· Tech-savvy with proficiency using software like Google Suite, Microsoft Office, etc.
· Ability to establish trust with participants, families, referral partners, and community stakeholders
· Valid driver’s license with reliable transportation for local travel

Pay: $60,500.00 - $70,000.00 per year
