Direct Care Worker/Home Care Aide

Duties

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and personal hygiene care.
• Support clients with mobility and transfers using appropriate manual handling techniques and equipment such as Hoyer lifts and patient transfer methods.
• Monitor vital signs, observe patient conditions, and report any changes or concerns promptly to healthcare teams.
• Prepare nutritious meals tailored to dietary needs and assist with meal planning and preparation.
• Administer medications following care plans and ensure compliance with HIPAA regulations to maintain confidentiality.
• Provide transportation assistance for appointments, errands, or community activities to promote independence.
• Maintain a clean, safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing living spaces.
• Document care activities accurately in client records, including patient monitoring data and progress notes.
• Support clients with specialized needs such as dementia care, Alzheimer's care, autism support, or neurodevelopmental disabilities through personalized approaches.
• Uphold residents' rights by respecting privacy, promoting autonomy, and fostering a positive atmosphere aligned with long-term care standards.

Qualifications

• Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred.
• Demonstrated ability to observe patients carefully and respond appropriately to their needs.
• Knowledge of care plans, medication administration, patient transfer methods, and vital signs measurement.
• Certification in CPR and First Aid is required; additional certifications such as HIPAA training are advantageous.
• Experience working with individuals with developmental disabilities, intellectual disabilities, or neurodevelopmental disorders is a plus.
• Strong communication skills for effective client interaction and team collaboration.
• Ability to lift heavy objects safely and perform manual handling tasks while maintaining proper body mechanics.
