RN - Home Health & Hospice

JOB SUMMARY:

Patient-centered Nursing Care and Case Management of Home Care and Hospice patients in the home setting.

Schedule:

• 0.8FTE (32 hours)
• Variable Hours
• Rotating Weekend Call

Compensation:

• $42.66/hr DOE
• Shift Differentials

ESSENTIAL FUNCTIONS, DUTIES, AND RESPONSIBILITIES:

1. Administer skilled nursing care and case management in the home to Home Care and Hospice patients as authorized by the physician. Develop, implement and evaluate the nursing care plan for each patient.
2. Observe patients for signs and symptoms of complications and report to the physician.
3. Be responsible for supervision of home health aides and LPN’s assigned to his/her patients.
4. Promote health and well-being of patients through teaching, supervising, counseling and appropriate preventative and treatment measures.
5. Have knowledge of and be willing to utilize community resources.
6. Work collaboratively with other healthcare professionals.
7. Take “call” on weekends, holidays and evenings on a rotating basis and responds to patient needed as appropriate.
8. Comply with Livingston HealthCare Policies and Procedures.
9. Provide current evidence-based care, education and support for hospice patients, families and caregivers.
10. Document care responsibly and accurately.
11. Be able to become competent in electronic medical record.
12. Track and record mileage on appropriate form.
13. Actively participate in Quality Improvement.
14. Accept accountability for own actions.

ADDITIONAL RESPONSIBILITIES:

1. Complete Livingston HealthCare annual competencies within assigned time frame.
2. Attend and actively participate in all Hospice Interdisciplinary, Quality Improvement meetings, department staff meetings, in-services and Home Care team meetings as assigned.
3. Be responsible for maintaining agency’s image of quality care by providing excellent, patient-centered nursing care.
4. Attend the first available Hospice Volunteer training class, if not already Hospice trained.
5. Give talks to community groups as requested in collaboration with Home Care/Hospice Manager.
6. Assist in annual Hospice volunteer training as requested.

QUALIFICATIONS (Required):

1. Current Montana RN license.
2. BSN, AD or Diploma
3. Good assessment skills.
4. Ability to function independently
5. Good communication skills
6. Ability to effectively function in fast paced environment with multiple interruptions.
7. Own vehicle for visits
8. Current Motor Vehicle License.
9. Current BLS

ADDITIONAL DESIRABLE QUALIFICATIONS:

1. At least one year of recent nursing experience, preferably in community health and/or care of terminally ill.
