Role Overview:
Evaluates and treats patients in home settings, addressing speech, language, cognition, voice, and dysphagia deficits. Demonstrates knowledge across age groups, developing individualized treatment plans, and adjusting intervention strategies based on ongoing assessments.
Key Responsibilities:
- Conducts diagnostic assessments to determine patient needs.
- Develops and implements personalized therapy programs.
- Provides direct therapy using various strategies and settings.
- Coordinates with medical teams to plan and modify treatment goals.
- Documents evaluation results, treatment plans, and progress notes.
- Communicates effectively with patients, families, and interdisciplinary team members.
- Participates in departmental program development and supervises students during internships.
- CCC-SLP from ASHA or active clinical fellowship, or valid state licensure.
- Preferred 1-3 years of experience in hospital, SNF, outpatient, or home health settings.
- Strong communication, collaboration, and documentation skills.
- Ability to work effectively with diverse patient populations and age groups.
