Role Overview:
Responsible for delivering physical therapy services, evaluating patients, developing individualized treatment plans, and implementing interventions to promote health and achieve therapeutic goals.
Key Duties:
- Evaluate patients through data collection from records, interviews, and observations.
- Create and adjust personalized treatment programs based on ongoing assessments.
- Provide direct therapy services in various settings, aligning treatments with patient capabilities and care plans.
- Document evaluations, treatment plans, progress notes, and reports accurately.
- Communicate effectively with patients, families, physicians, and team members regarding patient progress and needs.
- Collaborate with interdisciplinary teams and participate in educational and professional development activities.
- Active license as a Physical Therapist or Occupational Therapist, with lymphedema experience preferred.
- Ability to perform essential duties including effective communication, visual acuity, and teamwork.
- Preferred 1-3 years of licensed physical therapy experience.
