Role Overview
The Speech-Language Pathologist evaluates and treats patients with speech, language, cognition, voice, and dysphagia deficits in home settings. The Speech-Language Pathologist evaluates and treats patients with speech, language, cognition, voice, and dysphagia deficits in home settings. They develop and implement individualized treatment plans, establish and modify intervention goals, and provide direct therapy using appropriate strategies. The role involves conducting diagnostic assessments, considering age-specific factors, and ensuring therapy aligns with patients' physical, emotional, and intellectual needs.
Key Responsibilities
- Assess patients through formal and informal diagnostics to determine intervention needs.
- Develop, implement, and adjust personalized treatment programs.
- Deliver direct therapy, planning activities to promote health and meet treatment goals.
- Coordinate care with interdisciplinary teams, including family and physicians.
- Document evaluations, treatment plans, and progress notes accurately.
- Communicate effectively with patients, families, and team members.
- Participate in program development and supervise clinical internships.
- CCC-SLP from ASHA or active clinical fellowship certification, or valid state licensure.
- Preferred 1-3 years of experience in hospital, SNF, outpatient, or home health settings.
- Ability to communicate effectively, have adequate visual acuity, and work collaboratively.
