Licensed Practical Nurse (LPN)
Responsibilities:
- Provide resident-centered care in a skilled nursing and rehabilitation setting
- Administer medications and treatments accurately
- Monitor and assess resident conditions
- Document care and communicate effectively with healthcare team members
- Collaborate with RNs, physicians, therapists, and families to develop and support individualized care plans
- Support residents in short-term rehab, long-term care, and memory care
- Promote dignity, safety, and independence within a homelike environment
- Current LPN licensure
- Strong compassionate and patient-focused approach
- Excellent communication and teamwork skills
- Ability to work effectively with diverse resident needs
- Knowledge of clinical best practices and safety protocols
