Physical Therapist Assistant (Home & Community Rehab)
Provide compassionate, patient-centered rehabilitation services to seniors and individuals with disabilities in home and community settings.
Key Duties
- Implement treatment plans under the direction of a Physical Therapist.
- Deliver hands-on therapeutic exercises to improve strength, mobility, and function.
- Conduct gait and balance training to enhance safety and independence.
- Educate patients and families to support care goals and safe mobility.
- Monitor patient response and follow safety-focused care practices.
- Accurately document care provided and track progress in required records.
- Physical Therapist Assistant licensure/certification as required by applicable regulations.
