Responsibilities
- Support residents with daily activities including personal care, hygiene, etc.
- Monitor residents’ health by taking vital signs and observing behavioral changes to ensure safety and well-being.
- Assist with medication administration following prescribed protocols, ensuring accuracy and compliance.
- Engage residents in therapeutic activities, social interactions, and life skills development to promote recovery and community integration.
- Maintain detailed documentation of resident progress, incidents, and care provided using electronic medical record (EMR) systems.
- Support residents with activities of daily living (ADLs) such as meal preparation, cleaning, and household chores to foster independence.
- Collaborate with healthcare professionals, social workers, and family members to coordinate comprehensive care plans tailored to each resident’s needs.
Qualifications
- Prior experience in long-term care settings such as nursing homes, assisted living facilities, or home & community care is highly desirable.
- Knowledge of behavior management techniques.
- Ability to perform first aid and respond effectively in emergency situations.
- Experience working with individuals with history of substance use disorder
- Familiarity with EMR systems for accurate record keeping.
- Valid driver’s license for transportation duties if required; driving experience is preferred.
