Duties
- Conduct thorough patient assessments, including medical history, behavioral health evaluations, and substance use screening
- Develop individualized treatment plans in collaboration with multidisciplinary teams, including addiction counselors and healthcare providers
- Coordinate discharge planning and post-acute care arrangements to support sustained recovery efforts
- Manage patient records with strict adherence to HIPAA regulations and maintain accurate documentation of treatment progress
- Facilitate intake processes for new residents, ensuring smooth onboarding and comprehensive understanding of their needs
- Provide case management services that include utilization management, diagnostic evaluation, and patient service coordination within inpatient and outpatient programs
- Support behavioral health initiatives by integrating mental health counseling techniques and addiction counseling strategies into patient care plans
- Assist with medical records review and documentation
Requirements
- Proven experience working in behavioral health or substance abuse is highly preferred
- Strong background in outpatient or clinic experience is advantageous
- Knowledge of hospital medicine practices, physiology, medical terminology, and patient assessment procedures
- Experience with addiction counseling, behavioral health services, mental health counseling, and discharge planning processes
- Familiarity with HIPAA compliance standards and medical records management
- Ability to perform intake evaluations and conduct diagnostic assessments effectively
- Skilled in utilization management and case management within inpatient or post-acute care environments
- Excellent communication skills with the ability to collaborate across multidisciplinary teams in fast-paced hospital or clinical settings
- Demonstrated understanding of managed care principles and patient service excellence
