The CNA Team Lead supports day-to-day operations and patient care quality, acting as a vital liaison between the Senior Director of Clinical Services and the field staff. This role balances direct patient care with leadership, fostering a collaborative environment rooted in compassion and accountability.
Team Leader CNA:
The Team Leader CNA supports the department with the day-to-day operations of activities and the quality of patient care. Assists with staffing, performs patient care as needed and acts as liaison between the Director of Clinical Services and CNA patient care team.

Key Responsibilities:
- Guide CNA preceptors with new employee orientation and education in hospice care.
- Enhance communication with preceptors to ensure existing CNA’s receive support in the field.
- Coordinate CNA assignments with new patient admissions based on patient location.
- Oversee CNA schedules to align with timely visits and documentation.
- Supervise CNA visits and documentation content.
- Plan and organize monthly meetings.
- Reinforce monthly CNA assigned education to ensure completion.
- Identify and share ideas with leadership to resolve any CNA problems or issues.
- Responsible for payroll, PTO (including sick time).
- Perform timely check-ins with staff.
- High school diploma or equivalent required.
- Current CNA certification.
- Three years Hospice experience.
- Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements; and is in good working order.
