Lighthouse is hiring! We are seeking compassionate and motivated SCN Field Screeners to join our Social Care Network (SCN) team serving the Bronx community.
The SCN Field Screener will work directly with community members and Medicaid members to complete Health-Related Social Needs (HRSN) screenings and collect important information to help identify health and social needs. This position is ideal for individuals who enjoy working with people, are comfortable in community settings, and are passionate about improving access to resources and support.
Responsibilities
- Conduct HRSN screenings with Medicaid members using approved SCN screening tools and platforms.
- Engage members in a respectful, professional, and culturally responsive manner.
- Explain the screening process and obtain required consent.
- Collect and accurately document member information, demographics, and identified needs.
- Verify member information and Medicaid eligibility as required.
- Enter screening information accurately and timely into designated systems.
- Maintain confidentiality and comply with HIPAA requirements.
- Participate in SCN trainings, team meetings, and quality improvement activities.
- Meet assigned screening goals and productivity expectations.
- Conduct screenings at designated Bronx locations and community-based settings.
Qualifications
- High School Diploma or GED required.
- Experience in community outreach, healthcare, customer service, social services, or data collection preferred.
- Strong communication and interpersonal skills.
- Ability to work with diverse populations.
- Comfortable using tablets, computers, and electronic data collection systems.
- Reliable transportation preferred.
- Bilingual candidates strongly encouraged to apply.
- Must be dependable, organized, and able to work independently.
Compensation
- Paid per completed SCN unit.
- Compensation: $5.50–$7.00 per completed unit based on experience, performance, and quality standards.
