Job Overview

We are seeking an experienced and compassionate Care Navigator to join our growing Medicaid care management team. In this role, you'll serve as a guiding resource for members, helping them navigate the healthcare and social services systems and connect with the support they need. Your experience, energy, and reliability will empower members to access better health outcomes through personalized outreach, screening, and care coordination. As a Care Navigator, you'll be a first point of contact for Medicaid members enrolled in our care management program. Working from our Brooklyn office, you'll conduct phone-based screenings, identify members' health-related social needs, and connect them with the right community resources - helping people access support they might not otherwise know exists.

This role requires prior hands-on experience conducting Social Care Network (SCN) screenings and/or navigation, including handling reauthorization cases. We're looking for candidates who already know the SCN screening and referral workflow and can hit the ground running. Regardless of experience level, all new hires complete our paid training to get fully up to speed on our systems, tools, and workflows.

Duties

  • Conduct phone-based health and social needs screenings with Medicaid members using standardized SCN assessment tools
  • Manage reauthorization cases, including re-screening members and processing renewals to maintain continuity of services
  • Accurately document screening results and member information in our care management system
  • Make and manage referrals through platforms such as UniteUs (via PHS) and Channels360 (via SIPPS)
  • Identify needs related to housing, food security, transportation, and personal safety, and connect members to appropriate community-based organizations and services
  • Follow up with members to confirm they received and are using referred services
  • Meet daily and weekly outreach and documentation targets
  • Maintain strict confidentiality of member health information (HIPAA)
  • Work closely with the care management team to escalate complex cases when needed

What We're Looking For

  • Prior experience conducting SCN (Social Care Network) screenings and/or navigation is required
  • Experience handling SCN reauthorization cases is required
  • High school diploma or GED required; some college or healthcare-related coursework a plus
  • Working knowledge of health-related social needs (HRSN) screening and the community-based referral process
  • Familiarity with the UniteUs (PHS) referral platform a plus - we provide paid training on the tools we use
  • Strong phone and interpersonal communication skills
  • Comfortable with computer use, data entry, and care management systems
  • Organized, detail-oriented, and dependable
  • Bilingual a strong plus
  • Compassionate, patient, and genuinely interested in helping people navigate the healthcare and social services system

Schedule & Compensation

  • Full-time, Monday–Friday, in-person at our Brooklyn office
  • Paid training provided
  • Immediate opening

Pay: $20.00 – $25.00 per hour
