We are seeking a dedicated and compassionate SOMOS Certified Screener/Navigator to join our dynamic healthcare team. In this vital role, you will serve as a frontline connector for individuals seeking to get screened to determine if they are eligible for enhanced services which may qualify them for free food support, rental assistance and more.
Responsibilities
- Conduct screenings and eligibility assessment using Findhelp software.
- Assist clients with any question or concern they may have.
- Collect and accurately enter data into the systems while maintaining strict adherence to HIPAA and patient confidentiality standards.
- Provide culturally sensitive client communication in bilingual settings when applicable, ensuring clarity and empathy throughout the intake process.
Experience
- Prior experience working on Findhelp.
- Knowledge of the Social Care Network Program.
