Hospice RN Supervisor
Position Summary
The Hospice RN Supervisor is responsible for overseeing the clinical performance of RNs, LPNs, and Certified Home Health Aides (CHHAs) to ensure the delivery of high-quality, compliant hospice care. This role includes conducting supervisory visits, mentoring staff, ensuring regulatory compliance, and supporting interdisciplinary team collaboration. The RN Supervisor plays a key leadership role in maintaining excellence in patient care and clinical documentation.
Key Responsibilities
Clinical Oversight
Conduct routine supervisory visits for RNs, LPNs, and CHHAs in patient homes and facilities
Evaluate clinical performance, competency, and adherence to hospice standards
Provide real-time coaching and education to clinical staff
Ensure care aligns with physician orders and individualized plans of care
Compliance & Documentation
Review clinical documentation for accuracy, timeliness, and regulatory compliance
Ensure adherence to CMS and CHAP hospice regulations
Assist in preparation for surveys and audits
Monitor infection control and safety practices
Team Support & Leadership
Serve as a clinical resource for field staff
Support onboarding and training of new clinical team members
Participate in IDT meetings and care planning
Collaborate with the Director of Nursing/Clinical Director on performance issues
Patient Care Support
Provide skilled nursing visits as needed
Assist with complex symptom management cases
Support crisis care and continuous care when required
Travel
Travel to patient homes, assisted living facilities, nursing homes, and group homes to perform supervisory visits and clinical oversight
Qualifications
Active RN license in the State of New Jersey
Minimum 2 years hospice experience preferred
Prior supervisory or leadership experience strongly preferred
Knowledge of CMS hospice regulations and CHAP standards
Strong clinical assessment and documentation skills
Valid driver’s license and reliable transportation
