Summary: The RN Supervisor will provide comprehensive nursing services to persons living in the Jawonio Residences. Will oversee the health activities in the residences.
Essential Duties and Responsibilities include the following. Other duties may be assigned. Note that employees may not be assigned all duties and responsibilities.
• Conducts ongoing reviews of medical records specific to consumer health needs. Reviews records to maintain an accurate assessment of consumer’s health condition and quality of care received.
• Participates in quality improvement activities and ensures that records and documentation meet regulatory and professional standards.
• Develops Quality Improvement measures and Corrective Action Plans.
• Oversees the scheduling of medical appointments as prescribed or as deemed appropriate for the consumers. Serves as the Health Care Manager for consumers.
• Creating and Implementing Medical Care & Treatment Plans.
• Assists the LPN, as needed, by providing health care coverage, directing nursing care and administration of medications when needed.
• Performs supervisory functions including but not limited to coaching and redirecting, verbal, written counseling, initial training, periodic inspection, retaining and hiring. Contributes to the performance evaluations of LPN and direct care staff. Assists in providing health related education/supervision of LPN and direct care staff.
• Serves as a Liaison and Advocate for Service Recipients
• Oversee medication administration of all individuals and certifications process of all medication certified staff, in accordance with NYS Education Department. Department of Health & OPWDDD
• Maintains collegial relationships with physician and other health care providers when providing health care services.
• Participates in Utilization Review and DSRIP Projects
• Reviews & Obtains Consents and ensures compliance as per OPWDD Regulations
• Assists in the supervision of the health maintenance of consumers at multiple sites. Oversees nurses’ notes, lab reports, physician referrals and other documentation as required by agency/professional standards. Alerts other agency personnel as may be required regarding infectious/communicable diseases, internal and serious reportable incidences.
• Participates in Management meetings and Committees regarding the activities of the agency related to health care.
• Flexibility with schedule based on program needs.
• Participates in rotation of the Emergency on Call Schedule
• Submits Triage Reports
• Conduct Formal In-Service and Trainings on health-related topics.
• Additional responsibilities as assigned.
• Must cooperate and participate fully with Jawonio and/or Justice Center and OPWDD during an investigation or inspection, including making verbal and written statements as requested by the person(s) doing the investigation.
• Perform fit testing on medically cleared employees for N95 respirator masks.

Qualifications: To perform this job successfully, an individual must be able to perform each essential assigned duty satisfactorily. Education/Experience: Licensed by NYS to practice as a Registered Nurse, plus a bachelor’s degree from accredited Nursing program, plus three-year experience in similar setting. One year of supervisory experience preferred. A similar combination of education and experience will be considered.
Supervisory Responsibilities: This job has supervisory responsibilities.
Software: Working knowledge of Microsoft Office Suite, Smartboard presentations and ability to learn new software. Excel, Precision Care, Telemedicine, & EMR.

Additional Requirements:
• Daily access to an automobile is required. A valid New York State Driver’s License, ability to drive, and driving record acceptable for agency insurance coverage are required.
• Requires flexing regular schedule to accommodate operational needs.
• Frequent contact with consumers, employees, and supervisory management.
• Must be cleared by the Justice Center via fingerprinting and other background checks.
• Must be clear and maintain an acceptable record under the Medicaid Fraud & Abuse clearance process.
• Remain current and up to date with First Aide/CPR certifications.
