As an LPN, you will work directly with our developmentally disabled adults in their homes, supporting them in achieving greater independence, realizing their future potential, and contributing to their communities in a meaningful way by offering person-centered services.

Responsibilities include:
- Performing routine nursing care activities
- Supporting quality care for our adult individuals
- Assist with the preparation, implementation, and continuing evaluation of the nursing care plan

Qualifications:
What We Require:
- Associates Degree in Practical Nursing
- Valid NYS Licensed Practical Nurse (LPN) Certificate
- At least 1 year experience working within a similar setting
