What are the responsibilities and job description for the Meals-on-Wheels Case Manager position at LIFECARE ALLIANCE?
Job Purpose:
- This position ensures high-quality service delivery and compliance for LifeCare Alliance’s Nutrition Programs by combining client eligibility assessment with quality assurance oversight.
Key Responsibilities:
- Conduct home-delivered meal client intake and assessments to determine eligibility and service needs.
- Collaborate with the assessment team to address observation reports and process internal referrals to other LifeCare Alliance departments for clients.
- Ensure quality assurance and service monitoring to maintain program standards and compliance.
- Maintain accurate and timely documentation of client interactions, assessments, and service delivery processes.
- Provide support and guidance to clients, ensuring their needs are met in a compassionate and professional manner.
- Participate in team meetings and training sessions to enhance service delivery and professional development.
- Develop and maintain effective working relationships with clients, colleagues, and community partners to support program goals.
Required Education:
- Associates degree in social work
Preferred Education:
- Human Services or Community Health Worker certification is preferred.
Required Experience:
- Must have proven documentation and process management skills.
Preferred Experience:
- Minimum of 2 years’ experience working for a not-for-profit, community services, or government agency preferred.
- Additional preferred experience working with older adults and disabled/chronically ill individuals.
Required Skills and Abilities:
- Strong ability to conduct client intake and assessment for Home Delivered Meal services.
- Proficiency in collaborating with assessment teams to address observation reports.
- Competence in processing internal referrals to other LifeCare Alliance departments.
- Excellent quality assurance and service monitoring skills to ensure high-quality service delivery.
- Strong communication and interpersonal skills to effectively interact with clients and team members.
- Ability to maintain compliance with LifeCare Alliance’s Nutrition Programs standards and procedures.
