Position SummaryThe Wound Care Nurse Practitioner will provide comprehensive wound assessment, treatment, and follow‑up care in patients’ homes and assisted living settings. This role is ideal for an autonomous, clinically skilled provider who enjoys working independently, traveling locally, and building strong patient relationships.
Key Responsibilities
- Perform wound assessments, develop treatment plans, and provide ongoing wound management.
- Conduct patient visits in private residences and assisted living facilities within the Livingston, NJ region.
- Document clinical findings, treatment plans, and progress notes accurately and in a timely manner.
- Collaborate with caregivers, facility staff, and interdisciplinary teams to support optimal patient outcomes.
- Educate patients and families on wound care, prevention strategies, and overall health maintenance.
- Maintain compliance with all state, federal, and organizational clinical standards.
Qualifications:

Licensed Nurse Practitioner (NP) authorized to practice in the state of New Jersey.
