About the Role

We are seeking a compassionate, skilled, and self‑motivated Mobile Wound Care LPN to join our growing clinical team. In this role, you will travel to partnered long‑term care facilities to provide high‑quality wound care services to residents who depend on consistent, expert support.

This position is ideal for an LPN who enjoys autonomy, thrives in a mobile environment, and is passionate about improving patient outcomes through specialized wound care.

Key Responsibilities

• Provide comprehensive wound assessments and treatments in accordance with clinical guidelines
• Collaborate with RNs, wound care specialists, and facility staff to ensure continuity of care
• Document all care accurately in EMR systems
• Educate facility staff and patients on wound prevention and management
• Communicate changes in patient condition to supervising clinicians
• Maintain supplies, equipment, and professional standards while traveling between facilities

Qualifications

• Active and unencumbered LPN license
• Previous wound care experience preferred (training available for the right candidate)
• Strong clinical assessment and documentation skills
• Reliable transportation and willingness to travel locally
• Excellent communication and interpersonal abilities
• Ability to work independently and manage time effectively

Pay: $60,000.00 - $80,000.00 per year
