What are the responsibilities and job description for the Dementia Coordinator (C.N.A.) position at *Liberty Lutheran Services?
We are currently looking for a Full Time Dementia Coordinator/C.N.A. to join our enthusiastic team at Paul’s Run.

Shift / Schedule available:
- Full Time
- 8:30am - 4:30pm
- Schedule includes every other weekend
Key job responsibilities include:
- Provide leadership by serving as a role model for core competencies, maintaining professionalism, supporting a learning environment, collaborating with other departments, mentoring and guiding other household members.
- Create a life enriching dementia community by maintaining a sense of welcome through the continual elimination of institutional barriers.
- Enhance individual programming for the dementia population.
- Promote compassionate services among the team by offering emotional support to all and evaluating the successes and needs of the household.
- Promote the residents’ well-being and satisfaction by supporting resident-directed care, observing resident rights while using person-first techniques.
- Assist the team to maintain a quality work team by working side-by-side with the CNAs in the household and role modeling quality care and competence.
- Maintain a safe, comfortable and functional environment by assessing the household for potential hazards; implementing practices utilizing appropriate techniques regarding sanitation, infection control, fire safety, disaster preparedness and emergency care.
- Ensure the household is represented on all appropriate Healthcare councils/committees such as Resident Council, Food Committee and teams in the community.
- Assist in investigating and report all incidents and accidents, observe the team for safe work habits, and address issues.
Education and Experience:
- Eldercare/Long Term Care experience.
- Must have a Certified Nursing Assistant (C.N.A.) certification
- Knowledge of state, federal and local regulations and codes as applied to long term care.
- Demonstrate leadership abilities.
- Must have a passion for the quality of life of residents with dementia.
