Requirements

Location: On-site, Santa Ana, CA
Caseload: 25 Clients per CM
- Day to day:Provide referrals and linkages to primary and behavioral health services and other supportive services.
- Collaborate with referring organizations to coordinate guests care and ensure guests receive the services and support they need to achieve and maintain health, mental health, and housing stability.
- Assist with housing applications, complete supportive and subsidized housing paperwork, survey rental market for affordable housing.
- Advocate with prospective landlords and identify appropriate permanent housing options for guests.
- Provide ongoing information, referrals, linkages, and advocacy for all other identified needs
- Facilitate psychosocial rehabilitation groups for guests as required.
