Location: South Austin, TX (6800 Westgate Blvd)
Salary: $50,000/year (Exempt, Salaried)
About The Role
This Program Director position specifically supports our Host Home Program. The role begins with in-office training; however, long-term it transitions to a primarily remote position, with the expectation to work from the office at least one day per week.
You will oversee a caseload of approximately 50 individuals, primarily those receiving Host Home services, along with the oversight of one group home. You’ll work closely with the nurse assigned to each individual to ensure that all services, supports, and documentation are in place.

About The Role

Key Responsibilities
- Oversee the implementation of individualized, person-centered plans that prioritize health, safety, and personal development.
- Ensure compliance with regulatory standards and internal policies.
- Manage day-to-day Host Home Program operations, including a caseload of ~50 individuals.
- Maintain ongoing communication and collaboration with the nursing team to ensure proper service delivery.
- Provide oversight for one assigned group home.
- Visit the assigned group home a couple of times per month.
- Visit each assigned Host Home location at least once per quarter.
- Maintain accurate documentation and ensure quality of care for individuals served.
- Build and maintain strong relationships with host home providers, families, staff, and community partners.
- Participate in an on-call rotation and respond to emergencies appropriately.
- Travel to host home sites, group homes, community meetings, and other off-site locations as needed (mileage reimbursed).
- Bachelor’s degree in social work, psychology, sociology, or a related field.
- Experience in case management or a similar role strongly preferred due to the size and complexity of the caseload.
- Highly organized, detail-oriented, and able to manage multiple priorities in a fast-paced setting.
- A reliable, self-directed professional who is punctual, accountable, and comfortable working both independently and as part of a team.
- Strong leadership, communication, and problem-solving skills.
- Comfortable with an on-call rotation and capable of addressing urgent needs.
- Valid driver’s license, reliable personal vehicle, car registration, and auto insurance.
