This is a true regional field position with $110,000-$210,000 annual earning potential, a $15,000 Full-Time Sign-On Bonus, Monday-Friday scheduling, and no weekends or holidays.

Every House Gives You a Different Clinical Puzzle

One patient is recovering from a joint replacement.

Another has returned home after a prolonged hospitalization.

Another has fallen twice in the past month.

Another needs enough strength and balance to safely remain at home.

You'll see the stairs, furniture, flooring, caregiver support, fall risks, and everyday obstacles affecting your patient's independence.

Then you'll use your clinical judgment to determine what needs to happen next.

What You'll Do

• Complete in-home Physical Therapy evaluations and treatment visits
• Complete Start of Care assessments and admissions
• Develop individualized plans of care
• Address strength, balance, gait, transfers, mobility, endurance, and functional independence
• Identify fall risks and home safety concerns
• Educate patients, families, and caregivers
• Supervise and collaborate with Physical Therapist Assistants
• Coordinate with nurses, physicians, therapists, and the interdisciplinary team
• Monitor progress and modify plans of care
• Complete accurate and timely documentation and OASIS assessments

You Don't Need to Come From Home Health

We're interested in PTs from Acute Care, Inpatient Rehabilitation, Outpatient Orthopedics, Skilled Nursing, Transitional Care, Senior Living, Hospital-Based Rehabilitation, Geriatrics, and Home Health.

Home health and OASIS experience are preferred, but they aren't required.

We're looking for a PT with strong clinical judgment, organization, communication skills, and the ability to independently manage a productive regional caseload.

You'll need a current Minnesota Physical Therapist license, valid driver's license and reliable transportation, and availability for at least 2 weekdays / 12 visits per week.

Most importantly, regional travel needs to genuinely work for you. This isn't a position where you'll occasionally leave the office. Being on the road and providing care throughout Southern and Southwestern Minnesota is part of the job.
