Job Title: Certified Occupational Therapy Assistant | COTA

Company: Legacy Home Care / Home Health Care, Inc.

Location: Mankato, Waseca, St. Peter, New Prague, Le Sueur & Surrounding Communities

Schedule: Full-Time or Part-Time | Monday-Friday

Compensation: $82,000-$110,000 per year

Want to stand out from other applicants? Email Nikky Vogelgesang at nikky.vogelgesang@hhcare.net with your resume and tell us about your COTA background, what you're doing today, and what would need to be different about your next therapy position to make a move worthwhile.

More Freedom. More Variety. And $82,000-$110,000 to Do Work You Already Know.

What if being a COTA meant starting your morning from home, seeing patients throughout your territory, managing your own day, and spending your visits helping people accomplish things that actually matter in their everyday lives?

That's home health.

Legacy Home Care and Home Health Care, Inc. are hiring a Certified Occupational Therapy Assistant (COTA) to provide home-based therapy throughout Mankato, Waseca, St. Peter, New Prague, Le Sueur, and surrounding communities.

You'll have Monday-Friday scheduling, $82,000-$110,000 earning potential, mileage reimbursement, one-on-one patient visits, and the independence to work outside the same four walls.

If you've spent years in SNF, rehab, outpatient, senior living, or another facility setting, this could be a completely different way to use the skills you already have.

Why Give Home Health a Look?

  • $82,000-$110,000 annual earning potential
  • Monday-Friday schedule
  • Full-Time and Part-Time opportunities
  • One-on-one patient visits
  • Start your day from home
  • Territory-based scheduling
  • Independence throughout your workday
  • Consistent patient volume
  • Mileage reimbursement between visits
  • Cell phone reimbursement each paycheck
  • Free Education Hub access for continuing education
  • Licensure reimbursement
  • $250 annual scrub reimbursement
  • Medical and dental insurance
  • Life and disability coverage
  • PTO and sick time
  • 401(k) with employer match
  • Experienced OTR/L and interdisciplinary support

Trade the Therapy Gym for the Real World

As a COTA, you're constantly working toward functional goals. Home health lets you see exactly how those goals translate into someone's everyday life.

Instead of practicing a transfer somewhere else, you're helping someone safely get out of their own chair.

Instead of talking about kitchen safety, you're working in their kitchen.

Instead of simulating a morning routine, you're helping develop strategies that work in their actual home.

You'll work under the direction of an OTR/L while experiencing different patients, homes, challenges, and opportunities to help someone become more independent.

What You'll Do

  • Provide occupational therapy treatments in patient homes under the direction of an OTR/L
  • Carry out individualized treatment plans and interventions
  • Help patients improve ADLs and functional independence
  • Address strength, coordination, mobility, and functional skills
  • Reinforce adaptive techniques and compensatory strategies
  • Provide adaptive equipment and home-safety education
  • Educate patients, families, and caregivers
  • Monitor patient response and communicate progress to the supervising OT
  • Complete accurate and timely EMR documentation
  • Collaborate with the interdisciplinary care team

You Already Have Skills That Can Translate

You don't need years of home health experience to make the move.

We're interested in COTAs coming from Skilled Nursing, Inpatient Rehabilitation, Outpatient Therapy, Transitional Care, Senior Living, Long-Term Care, Hospital-Based Rehabilitation, Geriatrics, and Home Health.

If you're comfortable treating patients, building relationships, following an OT plan of care, recognizing changes, staying organized, and managing your responsibilities independently, your experience can translate well.

What We're Looking For

  • Graduate of an accredited Occupational Therapy Assistant program
  • Active Minnesota COTA license
  • Valid driver's license and reliable transportation
  • Strong patient communication and education skills
  • Confidence working independently in patient homes
  • Strong organization and time-management skills
  • Ability to collaborate closely with a supervising OTR/L
  • Commitment to helping patients maximize independence and quality of life

Your Next COTA Job Doesn't Have to Look Like Your Last One

You can stay in Occupational Therapy without spending every workday in the same building following the same routine.

Home health gives you the chance to get out into the community, work one-on-one with patients, and enjoy greater independence throughout your day, while still having an experienced clinical team behind you.

Add in a Monday-Friday schedule and $82,000-$110,000 earning potential, and this may be the kind of change that makes staying in therapy exciting again.

Want to stand out from other applicants? Email Nikky Vogelgesang at nikky.vogelgesang@hhcare.net with your resume and tell us about your COTA background, what you're doing today, and what would make your next position worth making a move for.
