Physical Therapist Assistant (PTA) | Home Health

Legacy Home Care
Long Lake, MN Full Time | Part Time
POSTED ON 10/9/2026
AVAILABLE BEFORE 2/5/2027

I’d make the next one about professional independence. The previous ad sells escaping the back-to-back therapy treadmill; this one sells being trusted to manage your day and work independently without losing PT support.

Job Title: Physical Therapist Assistant (PTA) | Home-Based Therapy

Company: Legacy Home Care / Home Health Care, Inc.

Location: Buffalo, Delano, Waverly, Watertown, Maple Lake, Annandale & Surrounding Communities

Schedule: Full-Time or Part-Time | Monday-Friday Only | Minimum 2 Days / 12 Visits Per Week

Compensation: Competitive Compensation with Strong Full-Time Earning Potential

Want to stand out from other applicants? Email Nikky Vogelgesang at nikky.vogelgesang@hhcare.net with your resume and tell us about your PTA experience, where you'd prefer to cover patients, and how many days per week you're looking to work.

You've Earned the Experience. You Should Be Trusted to Use It.

You know how to treat patients.

You know when they're progressing, when something has changed, and when it's time to involve the supervising Physical Therapist.

You shouldn't need someone looking over your shoulder every minute to do great work.

Legacy Home Care and Home Health Care, Inc. are hiring experienced PTAs throughout the western Twin Cities and surrounding communities who are ready for greater independence in their workday.

You'll take an established plan of care into the patient's home, provide one-on-one treatment, communicate directly with your supervising PT, and manage your responsibilities out in the community.

Independence Without Being on an Island

Home health requires confidence.

You're walking into the patient's home without a therapy gym full of coworkers around you. You'll need to assess how the patient is responding, adapt within the plan of care, recognize changes, and communicate when something needs the PT's attention.

But independent doesn't mean unsupported.

You'll have access to experienced Physical Therapists and clinical leadership when you need them while having the freedom to focus on the patient in front of you.

Why PTAs Consider This Opportunity

  • Competitive compensation with strong Full-Time earning potential
  • Full-Time or Part-Time
  • Minimum 2 weekdays / 12 visits per week
  • Monday-Friday patient visits only
  • No weekends
  • Consistent patient volume
  • Territory-based scheduling
  • Mileage reimbursement between visits
  • Cell phone reimbursement every paycheck
  • Free MedBridge Education Hub
  • Licensure and annual scrub reimbursement
  • Experienced PT and clinical support
  • Comprehensive benefits for eligible employees, including medical, dental, life insurance, disability, PTO, sick time, and 401(k) with company match

Take Your Therapy Skills Into the Real World

As a Home Health PTA, you'll work under the direction of a Physical Therapist while providing treatment where patients actually need to function.

You'll:

  • Provide physical therapy treatments in patient homes
  • Implement established PT plans of care
  • Improve strength, balance, endurance, and functional mobility
  • Provide gait and transfer training
  • Guide therapeutic exercises and home programs
  • Reinforce fall-prevention and safety strategies
  • Educate patients, families, and caregivers
  • Monitor progress and response to treatment
  • Recognize and communicate changes to the supervising PT
  • Complete accurate and timely EMR documentation
  • Travel between patients within your assigned territory

Home Health Experience Isn't Required

Your experience may come from Outpatient Therapy, Skilled Nursing, Inpatient Rehabilitation, Orthopedics, Transitional Care, Senior Living, Long-Term Care, Hospital-Based Rehabilitation, Geriatrics, or Home Health.

What matters is that you've developed the clinical confidence to work independently while knowing when to communicate, collaborate, and ask questions.

You'll need a current Minnesota Physical Therapist Assistant (PTA) license, valid driver's license and reliable transportation, strong communication and organization skills, and availability for at least 2 weekdays / 12 visits per week.

And keep your patient visits Monday through Friday with no weekends.

If you're ready to take the experience you've already built into a more independent therapy environment, we'd like to meet you.
